Testosterone panel – total testosterone, SHBG, and calculated free fraction. More precise than a single measurement.
The testosterone panel is a more precise version of the basic measurement. Most testosterone circulates in the blood bound to proteins – mainly SHBG and albumin – and in this form, it is biologically inactive. Only the free and bioavailable fraction has a real impact on muscles, bones, libido, mood, and metabolism. Therefore, we assess total testosterone along with SHBG, which allows us to calculate free testosterone. High SHBG – typical in hyperthyroidism, liver diseases, restrictive diets, or certain medications – can "hide" a real androgen deficiency behind a seemingly normal total result. Low SHBG, common in insulin resistance and obesity, works the opposite way. This panel is recommended for men with symptoms of testosterone deficiency whose basic test yielded a borderline result, for individuals monitoring the effects of lifestyle changes or therapy, and for athletes controlling recovery and the risk of overtraining. A full picture of androgen metabolism helps to avoid both overlooking a real problem and unnecessary treatment.
